چکیده

Dr. Vianello and colleagues understand very well that it is usually because of an inability to cough out airway secretions during chest colds, rather than because of chronic hypoventi¬ lation, that patients with neuromuscular disease develop acute respiratory failure (ARF). They deserve commendation for directing their attention to these patients' primary impedi¬ ment to extended survival. They propose using indwelling minitracheostomy tubes to facilitate secretion aspiration in times of need and note that hospitalization rates can also be decreased in this manner. It is not surprising that their approach can decrease the risk of respiratory failure and decrease hospitalization days, at least early on. However, although not mentioned in their letter, complications of minitracheostomy can be similar to those of full-sized indwell¬ ing tracheostomy tubes. They include tracheal damage, chronic bacterial colonization, granulation tissue formation, bleeding, diminished autonomous cough flows (because of air leak around the outer walls of the tube), and increased secretion generation. Furthermore, Vianello and colleagues note that "subjects. .who require hospitalization because of

Measurement of airborne influenza virus in a hospital emergency department.

Size-fractionated aerosol particles were collected in a hospital emergency department to test for airborne influenza virus. Using real-time polymerase chain reaction, we confirmed the presence of airborne influenza virus and found that 53% of detectable influenza virus particles were within the respirable aerosol fraction. Our results provide evidence that influenza virus may spread through the...

Spatio-temporal variability of aerosol characteristics in Iran using remotely sensed datasets

The present study is the first attempt to examine temporal and spatial characteristics of aerosol properties and classify their modes over Iran. The data used in this study include the records of Aerosol Optical Depth (AOD) and Angstrom Exponent (AE) from MODerate Resolution Imaging Spectroradiometer (MODIS) and Aerosol Index (AI) from the Ozone Monitoring Instrument (OMI), obtained from 2005 t...

Posture perfect: the role of positioning during bronchodilator administration with oxygen or heliox.

Over the years there has been great interest in treatment of patients with moderate to severe asthma presenting to the emergency department. To make a dent in these exacerbations, clinicians have modified label recommendations for short-acting bronchodilators with innovative delivery techniques, devices, doses, and frequencies of administration. Since 1935 the use of helium-oxygen mixtures (hel...

Aerosol transmission of influenza A virus: a review of new studies.

Over the past few years, prompted by pandemic preparedness initiatives, the debate over the modes of transmission of influenza has been rekindled and several reviews have appeared. Arguments supporting an important role for aerosol transmission that were reviewed included prolonged survival of the virus in aerosol suspensions, demonstration of the low infectious dose required for aerosol transm...
